The practice is registered and complies with the Data Protection Act 1998. Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the Act.
You may access your records which are held by the practice via the following options:
- Submit your request in writing
- Using one of the online services. To use Patient Access you must have an account for this service.
If, however, you require a copy of your medical records, please be aware that there is charge towards this service.
