We can only accept new patients who live in the practice catchment area. To see if you are within this catchment area, please use the map below.
You can register by completing a registration form (PRF1) which can be downloaded here (PDF, 229KB). As part of the registration process, we will require a proof of identity and proof of address.
When you register, we will ask you to make an appointment to see the practice nurse for a new patient health check. This will ensure that our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests/mammograms) are up-to-date.
